Lens data collection form
This form is developed to collect and store data of optics in a uniform format. The form is made as a Word document and can easily be downloaded to your computer and then copied or printed as much as needed.
To download the form, give a right mouse click on the link below, then click on "save target as" and then choose a directory on your computer where you want to save the form. If you prefer to open the form directly, just give a left mouse click on the link. This last option only works when MS Word is installed on your computer.
